Misconduct Reports
Referees are required to submit a Referee Report (formerly known as the “Referee Misconduct Report”) for any Send-Offs of players, team officials or spectators that occur in Region 325 play within 48 hours of the game. This same report is also now used for the referees to report any Serious Injuries which occurred during a game.

NOTE: Cautions issued by the Referee are to be reported on the back of the lineup cards; a separate Referee Misconduct Report is not required.
Incident Report Form
An AYSO Incident Report Form is used whenever there is a personal injury, damaged property, or threats of or actual physical violence surrounding an AYSO game, practice, event or property. The form should be prepared by the coach, AYSO Official, or AYSO Volunteer or by tournament or event staff members. Instructions are included within the linked document.
