AYSO provides supplemental insurance (Soccer Accident Insurance – SAI) for injuries to registered AYSO players sustained during AYSO activities.  See AYSO’s website for additional information. Note that if your player needed to be treated by a doctor then the Participation Release form (also on AYSO’s website) needs to be filled out by a parent/guardian or doctor if the injury was a concussion.

If your child will be walking, riding their bike or driving themselves to practices or games, you need to complete and submit a General Release Form (on AYSO’s website) to your child’s coach.
