Coach check-in will be Friday, April 17, 2026 from 6pm- 10pm Arizona time at the room above the snack bar at the fields 2315 Balboa Dr.   

Please be prepared with:

2 finalized, signed rosters from your club/organization, not handwritten and not gotsport unless stamped by your club/organization, preferably in numeric order
Guest Player Forms 
Medical Release Forms for all players, in numeric order
4 Game Cards/Line Up Cards, filled out in numeric order. 


The players listed on the game cards must match the approved roster and guest player forms.
Fillable, Printable Line Up Cards Please print on legal size card stock.

Each coach or assistant coach must also check in entire team and coaches with laminated photo ID cards or roster with photos, official stamped roster (stamped at coach check-in) and a stamped game card at least 30 minutes prior to FIRST game and each player will be issued a tournament bracelet. The bracelet issued and must be worn for each game.  A player without a bracelet is not eligible to play.
