How to print team player release forms.


Emergency release forms of all players MUST be present at all games and practices.
The team coach has the first set of release forms at all times with him.
Assistant coach (AC) and team manager (TM:certified volunteer parent) should have a copy.
If the coach is temporarily absent he should leave (copy) of coach binder with AC or TM in charge.
